Texas RN License
Negative TB test
Signed statement of history of no misconduct

CERTIFIED NURSING ASSISTANT

Certified Nursing Assistant for the state of Texas
Completion of the orientation training
Signed ‘Statement of No Misconduct’
Negative TB test
Provide the agency with true statements regarding qualification requirements

COMPANION SITTER

First Aid/ CPR
Read and write English fluently
Ability to follow verbal and written instructions
Completion of training or pass competency assessment as appropriate
