Who is Infinity Group Health?
Infinity Group Health is a physician-led care coordination organization committed to improving outcomes for high-risk patients through strategic house call services and structured care transitions.
Through our MSO partnerships, we support contracted physicians and nurse practitioners who provide in-home medical care to patients who need consistent, accessible, and proactive clinical oversight. Our focus is simple: deliver the right care at the right time in the right setting.
A Strategic Care Transitions Partner
IGH partners with hospitals and healthcare agencies to strengthen post-acute discharge planning and reduce avoidable readmissions. Our Care Transitions Program ensures that patients do not fall through the cracks once they leave the hospital.

Infinity Group Health leverages strategic partnerships with licensed Medicare Agents to ensure patients are enrolled in insurance plans that align with their medical needs and provider networks. Services Offered
Our mission is to alleviate burdens faced by individuals and hospital organizations who lack education on resources available to enhance and improve healthcare outcomes upon discharge from hospital.
Housing
Our housing initiatives offer shared living arrangements to individuals who can no longer safely live independently. All of our assisted living facilities and residential care homes are overseen by physicians which significantly improves quality of life and preserves dignity.

Healthcare and Medicare
We offer a specialized Care Transitions Program in partnership with assisted living facilities and local hospitals. Our MSO contracted physicians and Nurse Practitioners aids in reducing hospital readmissions while providing post-operative patient education and enhancing patient outcomes.
In addition, we offer dedicated Medicare Agents to assist with complex health insurance issues such as prior authorization rejections and more.
